Bank staff not able to debit bank account

Bank staff said that they are not able to debit the bank account of a customer, though the customer has not received any notification. The bank account does not have any new deposits or transfers. What could be the reason why the bank cannot debit, and who initiated this action?

Am not sure I understand though, but I think it might be due to network issues or just some sort of error.

The bank only debits the bank account if a deposit transaction occurs either by the owner or from someone else, but if the address is wrong such as the wrong account number then the deposit will be delayed or go to someone else's account.
