Change in date of birth of nominee

While making a fixed deposit online initially, the date of birth of the nominee was specified. Yet the next day, the investor went to make another fixed deposit, the date of the birth in the nomination data was deleted. What could be the reason for the problem

I think the best cause of action to take would be to make a complain to the bank directly, the fault or mistake should be from them.
