Some businesses have systems and structures, but those systems and structures don't work. Why?

I know of many business organizations that have set very nice systems and structures. But surprisingly, those systems and structures don't work. The business is still not properly organized.

Why would a business institute systems and structures but they just don't work?

Why some businesses with good structure on paper can't translate those structures in practice is simply because they don't have capable hands to man the system and run the structures.

Most business systems fail because owners focus on the wrong metrics, ignore cash flow reality, , spend more to acquire customers than those customers are worth, and hire the wrong people.
