Why do smartphone chargers stop working?

I have noticed that a large number of smartphone chargers stop working after some time. Does erratic power supply or surges in power supply damage the charger or is there any other reason why the smartphone charger stops working? How to prevent damage to the chargers?

How are users misusing the smartphone chargers which make them malfunction? The chargers are connected to the power supply, and the smartphone, there is nothing the user does to damage them.
