How do you detect an unhappy customer as they leave your shop after patronizing your business?

There are some customers that actually come and patronize a business and are not satisfied even when they have made the purchase. They come into a shop or business place, actually buy and pay for the product or service, but are not really happy as they leave.

How can a business detect unhappy customers as they leave the shop?
 
I simply ask questions to get feedback. I also look at the countenance of my customers. Furthermore, I chat them up to know their experience with me and get to find out if there's anything they'd love me to improve on.
