Incorrect government records for tax id

The fathers name in the government records for tax id was always correct, only this year, it appears that there is an error in the fathers name due to which the KYC is getting rejected. I have not submitted any documents with the incorrect name. Does anyone know how the fathers name in the government records could have changed from correct to incorrect ?
