Has your boss ever told you to do something unethical? Did you do it? Did you report your boss to HR or management?

With all the jobs I have had so far, I haven't had any bosses try to ask me to do unethical things, so was curious on what others have experienced.
 
My dean did an unethical act. I was a newly hired teacher in a private university teaching Physics and math, but she dared to request me to buy cigarettes and mail her letters for her covet. I was shocked because I was 20 at that time and felt excited to have started work but my chairman upon knowing it called an urgent meeting secretly for our dean's removal for unethical acts. It was revealed that it was not only me abused by that dean, but many and all newly hired teachers. The university president upon knowing it, advised her to resign.
 
I would not do anything unethical. I would be held responsible for it and not the boss when it backfires.
 
A staff in the school that I taught refused to participate in examinations malpractice to help the students in an external exams. The boss directed he helped the students with answers. He refused. And nothing was done to him.
